Wordpress forms - display in back end

To begin with, I have a wordpress site with 2000 users [employees], their
username = surname+name
password = employee number
On the landing page you can choose 2 options, "register device" and "log your speed", these do not relate to each other, but you need to view the stats for both in the backend of wordpress.
1. Register device

On the Frontend the User will fill out 5 fields:
1. Name
2. Employee Number
3. ID Number
4. Device number
5. Status
Once complete it must say "thank you for regestering"
In the backend under "register" it must list all the entries in a list with the headings : "Name", "Employee Number", "ID Number", "Device number", "Status"

2. log your speed

Frontend will have 2 forms "employee number" & "speed", ...... the site must also track how many times that employee number has submitted and his average speed
Once complete it must say "thank you for your entry"
In the Backend, under "speed tests" it must list all users [employees] with the headings:
"user" | "employee number" | "times submitted" | "average speed"
and in the top right corner it must list "Total average speed"
