Dimension on products and design for OsCommerce

For a customer, I need a module for OsCommerce that can do the following:

The administrator can create a product, and enter what the price for that product is pr. mm,

The customer can then order a product, like a window, wooden plate, or anything else you may sell in your shop, by entering the dimensions.

The product page should look something like this:

This mod, should only apply for products where the administrator wants it, and not for all products.

For same OsCommerce shop, I need a design. Logo and more will be supplied by me.

This project has been listed 2 times now, and none of those supplier has finished the project.
My client has waited since november 2010.
